In the other part, the possibilities and skills of Etiopathogenic Research are advanced a lot and we will not be taking in consideration, in this articles, the neurobiological and pharmacological developments.
Introduction
Particularly: questioning, which comme first: Does Oedipial Conflict remains Central...Probably in the organization of the drives and the related phantasmatics But, children known early so much about the aspects of the sexual question...Great Anger Crisis, fit of Rage, with possible self-offense or suicidal attempt, Instinctual Disorders, such as anorectic/bulimic states, acute persistent depression with rapid mood fluxtuation or with loneliness’s feelings, strong Identity concerns are more linked to the Impact during the First years of original Fantasies/Fantasmatique as discovered and conceptualized by M. Klein and his epigones. These disorders are also to be understood at the level of the Realistic Function and concerned by the Formatting of Psychic Defense Mechanisms of whose prevalence can remain prrimitive such as cleavage, denial, projective identification [1-3].
p align="justify"?Psychopathological Configuration and Clinical Expression the deepening of the knowledge of the levels of the psychic development of the early child -of the baby - is recent and brought new knowledge, which makes possible to understand different types of emotionnally considerable fantasmatization. The pionners in pedo-psychiatry are mentioned. Psychotherapies for children have been able to be developed and those for adults have been possibibly transformed for better [4-6]. p align="justify"?Nosology references/ nomenclatures C.I.M. 10 and D.S.M. V are to be considered as well as the specific scales. Disharmonic features of the youngs may lead to New Aspects of disturbancies later, which were not taking in consideration before : such as Personality in Mosaic, may be typical States Disorders with possible Entangled Symptomatologies : Recall : Identity ‘s Troubles, Weakness of Self Esteem and Bad emotional control Aloneless Feelings Depressive and Anxious States Addictive Behaviors Irritability Different types of possible abrupt and angry Actings, sometimes Transgressive Self Injury or damage Reactivity depending of the Ego Strenght, of the control of Emotions and of the type of the twelve Defense’s Mechanisms implemented : More Usual : Sublimation, Repression, identification, rationalisation, déplacement, More used by Borderline subjects: cleavage, projection, Projective identification, (described by M. Klein during the schizo-paranoid position), isolation, Denial but not forecclosure, reversal into its opposite [7-10]. p align="justify"?Since the works of S. Freud and his epigones, including J. Lacan, since « Neurosis, Psychosis and Perversion », the oedipal conflict was central in the conduct of Psychotherapy and Analysis. We think notably to the « Five Psychoanalysis » and specially to the « Wolfs’s Man ». The Family and the social context, including Alienists at that time and Old Psychiatric Hospitals conformed and confirmed Nosological Repertoire Expressions in the field of the convention and the Legal Framework. Already, innovative works of Mélanie Klein (who discerns a phase schizo-paranoïd and precocious depressive during the first years), Anna Freud, J. Bowlby (that one with his conception of the impact of the attachment) and later, the ones of Spitzer and his colleagues and epigones, who will clarify new clinical conceptions and help to a conceptualization of a new nomenclature. (D.S.M.) The reflection about the importance of the first fantasms of early years of the psychic and mental development is highlighting from 1940 [11,12].
Changes and Great Transformations of Environment - Rôle of Societal Events
The Two World Wars, the upheavels of mass, the scrambling of ideologies promotes and facilitate the new conception of the life’s conditions. The extension of the existence due to prophylaxis and medical treatments ‘s progress leads to a differentiation/ separation of elders from their children and grand-children, finding by themself specific activities. All that induce an evolution towards a nuclear family, sometimes disssocieted, including separation, even unable for education with necessity of foster family or institutional’s « family ». The new Society changes the conceptions, sometimes ethical and forward-looking, the quality of the relationships. notably due to the great technological, virtual, digital, economic progress. The modifications or even breaks in configuration of the family are becoming a reality : more rarely conform but rigid, more often « synchretic », dissociated, reorganized...Of substitution : type of forster family or institutional one, when educational skills fail [13,14].
The expension of the exchanges between the different geographic cultures favorised by easy travels open up to unexpected perspectives and the use of internet, which should permit to send mails so quickly, modify the states of mind and the desiggn of uses. Nethertheless, about Great Adolescents and Youngs adults, particularly the ones who have suffered of Childhood’s Disharmonic features, with regard to these subjects, the history of whom we have recconstructed, both clinically and eventally within the framework of care or administrative or judiciai expertises ...what could have happened : they have often evolved towards mosaic personality structures and labile emotional organization [15].
At this level, the school monitoring and the educational course, less rigorous and in-depth as during the past old time, contributed some of them to have a great imagination, even awesome but not enough intellectual ressources and conceptual handling to guide their achievement and to help them to control their pulsional emergences, which can lead to possible angry actings. And even possibly arouse the prevalence of instinctual register affects such as anorexia/boulimia and various pharmacological, toxic, sexual or at /play/games addictions. The disruptions could here reach the degree of blind violence, which would be summed up by the ego equation : I or the other one [16].
Certainly, they can get involved in a search of the beauty but without real esthetic sense. Their life is marked by paradoxical behaviors and even chaotic events, so that they appear stable in their in the instability behavior. We could evoke in an exemplary way some artists as, for instance, famous singers, participants of the Factory in Brookling or, in literature Scott and Zelda Fitzgerald [17].
New Therapeutical Perspectives
During the last decades, the Psychiatric Resarch was developing about the early Realistic Function, that one being before the oedipial conflict and able to involve in the structuring of personality. That’s why it’s given now a priority at the psychogenetic and exploraty’s levels about the étiopathogeny, which later induce clinical diversity and heterogenity and, naturally towards an approach of the constitution of Identity. In that way, taking in consideration the possibility to had relationships with babys and very youngs children brought essential observations and permit great discovers. The tests are always greatly appreciated and help epidemiologic study [18].
The pedo-psychiatrists can follow finely the psyche’s development of the young, his childhood history, his maturative evolution which may include new different psychopathological expressions and formatting in their affective, emotional registers. But also, in their comportmental and cognitive dimensions. All that will caracterize their mental equipment and use of new models of therapy in individual or institutional fields. Listening therrapies, both youngs and families, are the most common, sometimes with the help of various mediations. Aiming to develop the potential for creativity is positive. We don’t neglect those with behavioral aspects The narration of the dream is always considers, as well as the listening to history and stories by the parents. The potential for expression may be aroused by the games, (D.W. WINNICOTT) by the drawning and the painting and the types of appreciation of interrelational capacities and skills in the poursuit in the context of team in group’s therapies.
It’s Necessry to Take into Account Different Kinds of Therapies
The Good Way is really the Empathic and sometimes inductive Listening, including some wise advice. Admitted that from time to time, pharmacotherapic help can be useful, sequentially bearing in mind that the use of BZD anxiolyics carries a risk of drug dependence. In regard to hyperactive hyperkinetic children, to anxious adolescents or conversely turbulent, sometimes tempted by addictive behaviors ones, a temporary treatment may be necessary.
That’s also the cases of disturbances as anorexia/boulimia. Finally, oncerning depressive states, aloness feelings or subjects having oddities without reaching the gravity of an entry into psychosis or especially in schizophrenic disturbanccies. Moods swings can be regulated by the normothymic drugs. But it’s also particularly useful to realize an psychotherapicc approach of children, who are inhibited, mutic, reluctant or provocative, or even presenting with hyperactivity an inappropriate comportmeent. Attention disorders require complex adjustments, sometimes with the help of a speech therapist when they are associated with oral or dyslexic ideational dysfunctions. The impact of a stale or hostile environment should not be underestimated
The Consequences of Abandonment
We present some observation in another paper. We have to highlight that these circonstances could be imaginary due to an attitude of withdrawal hindering in the quality of interactions with close adults or real. We will descrive illustrative observations in another article.
Conclusion
The rapeutic Goal is to take in consideration what happen, when the subject was the baby or the Young.
a) Gaining the confidence of the children or of the teenagers is more especially essential when it comes to youngs people :
b) Sometimes intricate, entangled sociopathic traits needs arrangements and the establishment of a reflection and educational and care group.
c) Confidence will also, sometimes, allow the revelation to toxins addiction, to harassment as well as physiquely abuse or the fact that they are victim of sexual aggression, with affective disqualification.
This discovery requires urgent action, these pedophilic acts seriously hampering, hindering the emotional future of the youngs and their flourish development.
